چکیده
Clinically the low wear of self-pairing aluminaceramic type BIOLOX (Feldmühle/CERASIV) in the self-locking AUTOPHOR and cemented XENOPHOR prosthesis (Osteo AG) was confirmed radiographically after 10 years (29) and recently after 20 years (33) of clinical experience. In laboratory measurements of retrieved prostheses (chiefly after infection or aseptic loosening) the low wear rate of ceramic self-pairing components was also confirmed by Mittelmeier et al. (28, 30), being at the socket 5,4 μm and on the head 2,6 μm/year in average. This results were in the range reported by Boutin and Blanquaert (3) with a wear of 9 μm/year on the head and 6 μm/ year on the socket in average. Dorlot (12), found a wear rate of 0,025 μm/year as the normal wear behaviour of firm well positioned alumina ceramic components in self-pairing design, pointing out, that higher wear-rates are caused either by false positioning of the components (primary or secondary due to loosening) or insufficient material properties of the alumina-ceramics. Dorlot’s (12) results could be confirmed by Walter’s (41) investigations, reporting wear rates in a range between 0,025 μm/year and 5 μ/year in well positioned cases.

Fracture of ceramic components as a serious failure after total hip replacement with ceramic-on-ceramic bearings
Introduction Ceramics have been introduced in total hip arthroplasty for several advantages including minimal friction, low wear, biocompatibility, no ageing problems and no problems related to the systemic increases in serum metal concentrations. In the first generation of ceramic components introduced in the 1970’s the risk of fracture was substantial, reaching up to 13.4 %. After introducing...
متن کاملConcepts of the Modern Ceramic on Ceramic Total Hip Arthroplasty and Early Results
We reviewed the results of 70 total hip replacements utilizing a modern ceramic on ceramic total hip prosthesis in 60 patients. The age of the selected patients ranged from 25 to 76 years (mean, 46 years). The diagnoses at the time of implantation were osteoarthritis (33 patients), avascular necrosis (29 patients), developmental hip dysplasia (four patients), rheumatoid arthritis (one), and pos...
متن کاملRetrieval analysis of alumina ceramic-on-ceramic bearing couples
BACKGROUND AND PURPOSE Ceramic-on-ceramic (CoC) bearings have been in use in total hip replacement (THR) for more than 40 years, with excellent long-term survivorship. Although there have been several simulator studies describing the performance of these joints, there have only been a few retrieval analyses. The aim of this study was to investigate the wear patterns, the surface properties, and...
متن کاملAlumina-on-Polyethylene Bearing Surfaces in Total Hip Arthroplasty
The long-term durability of polyethylene lining total hip arthroplasty (THA) mainly depends on periprosthetic osteolysis due to wear particles, especially in young active patients. In hip simulator study, reports revealed significant wear reduction of the alumina ceramic-on-polyethylene articulation of THA compared with metal-on-polyethylene bearing surfaces. However, medium to long-term clinic...
متن کاملAlumina ceramic-on-ceramic total hip replacement with a layered acetabular component.
A modular layered acetabular component (metal-polyethylene-ceramic) was developed in Japan for use in alumina ceramic-on-ceramic total hip replacement. Between May 1999 and July 2000, we performed 35 alumina ceramic-on-ceramic total hip replacements in 30 consecutive patients, using this layered component and evaluated the clinical and radiological results over a mean follow-up of 5.8 years (5 ...
